Exhaust then remap

Right, I've always got an exhaust followed by a proper remap at the same time.

This time I'm thinking about putting on a cat back milltek but not changing the ecu settings.

Has anyone done this and how did the car adjust?

Is this a very bad idea or should it be OK as the car will adapt to the exhaust?

I would say it should be fine. But worth getting a map at some point. But then it's always worth getting a map ;)

A cat-back will usually not require an ECU remap to see gains.
However, if you're doing downpipes and cats, then its recommended to get an ECU remap to realise the true potential.
eg, MTM only do one remap code for the RS6, whether you have a standard exhaust, or a cat-back sports exhaust.
But if you have the downpipes done, then they use a different map.
