some of you may or may not know that the Miltek system for the S3 consists of only cats and cat back - no downpipes or manifold (I am assuming manifold and downpipes the same thing?)

JabbaSports have their own manifold, as do MTM, and AMD have been advertising theirs as coming soon for the last year

Question: Is a new manifold required for a new Turbo, or is it a significent performance enhancement by itself, either with a Miltek or standard exhaust system.

My understanding for your question can relate to the RS4 and assume a similar answer to the S3

Basically because of the way the exhaust manifolds have been made there is a maximum amout of gas that can flow through them, on an RS4 it is impossible to get more than 476bhp through the manifolds, so that is why you upgrade tehm. Obviosuly you would do this at the same time as fitting hybrid turbos for maximum effect

But i assuma and am sure that you can refit the existing turbo, but whether you would get much gain i am not sure, probably just spool up quicker?

Manifold goes between head and turbo, downpipe from turbo to cat, exhaust goes from cat.

As above, you change the manifold to get better air flow.
Not 100% sure but I think the manifold is integrated into the casting of the turbo on the stock units ? can anyone confirm that ? All KKKs I've seen have been this way.

In other words you'd only replace the manifold if you are going to change the type of turbo you run, i.e. Garrett, IHI etc.
