New clutch slipping after 3K miles of use

Hi Gents, I recently had the clutch replaced around 3K miles ago. Unfortunately the symptoms which drove me to replacing it are now back.

1. Jumpy take off in 1st.
2. Occasionally slips at high rev changes.
3. Clutch only engages when pedal is almost fully out.

The car has never been launched above 3K revs nor has the clutch being ridden.

Any suggestions on what might be causing these symptoms would be very helpful.

Did you see what condition the last clutch was in after they took it out. I had a similar problem and it ended up being a seal that had gone and so leaking gearbox oil onto my clutch plate.
