If the cabin isn’t getting warm then it could be that the thermostat isn’t opening. This would result in excess heating around the block and possibly localised boiling. This would suddenly increase the pressure and coolant will be ejected via the expansion tank cap. I’ve had this on other cars.

This is exactly what’s happened to my wife Toyota Alphard and presented the same issues you’re talking about Nick. 👍
