Cam Adjustment intermitent and battery Voltage

Hello
I ´ve been logging this error for some time.
Used to clear to solve for a long time
Now everyday I get this.

Tested the cam adjustmente and after 2000 it turns ON as expected.
But riding hard the car in high rpm, most of the time cam adjst is OFF.

Besides, logging Voltage , I can see values from 13.7V to 11.9 V.
Anyone has logged Voltage and there´s all this range in the normal operation or it should be steady at 13.7 V ?

A faulty ground could generate all this ?

The last...is it "normal" change the cam adjst sensor in a 40000 miles car ?
