Nope - still alive!!!
Credit to the garage - they've worked out what it is, and even called me down to see what they'd found first-hand.
Seems like they found why the timing was out on the 'correct' settings and therefore why the adjusted setting had been made in the past with the paintmark.
I'll let you know once they've done the next stage and confirm it's definitely what they think it is. Everything they've discovered and said all makes perfect sense and so the logic is good, and although the fix is not going to be cheap (what is!) it will make the best perfect again in theory!
Should have it back late next week and will let you guys know. Useful for future readers down the line also no doubt.

Hi Guys
It turned out to be the guides on the tensioner for the cam chain on bank A.
These guides are made of PLASTIC (yes, I know!) and so not really up to the job IMO.
The bottom guide had completely snapped off, making the chain slack, throwing off the timing on bank A (cyclinders 1-4).
So, whilst popping in to take a look a couple of days back I asked them to remove the top of bank B whilst I was there to double-check that side. The top plastic guide was broken on Bank B also

Where the plastic has disintegrated, the chain was sat of the body of the tenioner itself and the chain had wore considerable 'grooves' into the body of the tensioner. It's just luck that the chains did not snap!
Also explains the rattle, as the chains were loose.
So, both tensioners and chains replaced (ouch!), cambelt set to factory position (ignoring paint mark), and all put back together. Also got them to drop off the sump and check for plastic pieces - which they found!
The result - car runs sweet-as with no fault codes or engine lights coming on. The downside - a big bill.
Why do they make such an important component in plastic?? Even the brand new replacements are still plastic. Grumble.
Still - if the chain had of snapped it would have been a much bigger bill!!!

Sounds good. How many hours did they charge you for the job?
I think the lesson here is that when your cam chain starts making noise then it is *not* safe to ignore it.

Yes, I was quite shocked. I thought the chains were rock solid. But the chink in that armour is plastic guides.
The job was priced at (from memory) 7-8 hours per bank. The cambelt was done three times in the end, but charged only for once.
