I seem to have the same problem........
It even displays the incoming number in the DIS!

I don't have a Multi function steering wheel, so I have no button to push to answer the call.

Not an RS4 mind, I have a B6 S4 with RNS-E.

ive had a similar problem with my parrot ive been trying to install. apparantly the audi head units need a feed from the blutooch receiver (in my case the parrot) into the 4th iso block (red vertical one) for it to be able to output the sound. This is cos the audi stereos work on low level outputs and the y harnesses with the parrot supplies a hogh level input to the stereo, which it does nothing with.

don't know if that applies to what you chaps are doing?

Do you mean no ringtone, or no audio when in a call?

It is possible to turn the volume down to zero, so try ringing your mobile and then turning up the volume control on the radio/RNS-E and on the phone - both have to be turned up.

When the phone rings all you have to do is press the big button on the RNS-E to answer the call - assuming your phone is paired with the car.

On reflection, I'm guessing that the fact that I have taken off the factory fitted cradle (the holes that this left are another matter) and have not connected the wires to the RNS-E may be the problem, no provision on the Kufatec loom that I ordered.

P.S. sorry if I'm hi-jacking this thread Declan, it may be related. :)
